Authorization for Use or Disclosure of Health InformationBefore forwarding your application to your appropriate agency, please use the following checklist to ensure the packet is complete. The application will not be reviewed unless it includes all of the following*:
- *Copy of Certificate of Degree of Indian Blood (Mandatory)
- *Signed Consent for Medical Services (Mandatory)
- *Drug and Alcohol Free Campus Contract and School Policies
- *Birth Certificate for all students applying for the 12th grade
- *Recent IEP for students requiring Special Ed services
- Immunization records
- Physical Exam
- Treatment Discharge Summary, Aftercare and Counseling Information
- If on probation, all records (court orders and PO reports/recommendations)
- School Leave Permission and Audio-Visual Release
- Custodial Parent/Guardian Information
- BIE McKinney-Vento Enrollment/Referral Form
- Home Language Questionnaire
- Custodial information and court documents showing custody
- *School transcripts – Please remove the request form from the application packet, sign and forward to the last school attended. Include any unofficial copy of your last school transcript. The application will not be reviewed if school records are missing. If Special Education services are required, a copy of the latest IEP must accompany the application.
- List of schools attended
- All consents for all information must be signed for student to receive services
- Consent for student participation in surveys and other activities
